9 staff disciplined over Savita case

  • Nine staff involved in the care of Savita Halappanaver at University Hospital Galway (UHG) prior to her death in October 2012 have been the subject of disciplinary action, the HSE has confirmed.

    The actions of 30 staff members were considered in relation to the controversial case and of these, 21 ‘had no case to answer' and were therefore not disciplined. Those 21 staff members were advised of this decision ‘some months ago'.

    "The disciplinary process in respect of most of the other nine staff is now complete. In some cases pre-procedural informal counselling with training and mentoring was applied, in others written warnings were issued. The process is still underway for a small number of staff," the HSE said.

    Mrs Halappanavar was 17 weeks pregnant and was miscarrying when she attended UHG on October 21 - she died a week later from a serious infection after she was refused an abortion during the earlier part of her stay.

    Three separate investigations were carried out into her death and in November 2013, the West/North West Hospital Group (formerly the Galway/Roscommon Hospital Group) ordered a review of the actions of all staff members involved in her care.

    The names of these staff members are not being released. Meanwhile legal action by Mrs Halappanavar's husband, Praveen, against the HSE is still ongoing.
